115學年貴儀中心校貴重暨共同儀器新購置/擴充升級提案

提案流程說明：
欲提案由”本中心”新增儀器或擴充”本中心現有”儀器規格者
(1) 提案之項目需屬於”全校使用”，系所(教學)、中心、實驗室或個人等用途則不屬於本次受理範圍。
(2) 請先至研發處貴重暨共同儀器中心網頁查詢本單位既有之儀器項目。
(3) 填寫「慈濟大學校貴重暨共同儀器提案申請表」。
(4) 將申請表(word)、估價單及佐證資料以電子檔寄出聯絡人。
(5) 印出紙本申請表經共同申請人、單位主管及院長簽核後繳交至勤耕樓D603貴重暨共同儀器中心。
(6) 申請擴充本中心現有儀器者，請務必確認規格符合現有儀器，以免造成購置後無法使用。
(7) 請提供至少三家參考廠商之型錄電子檔及估價單，另提供場地改裝、水電配置等儀器安裝或維持正常運作所需之其它費用，提案前請先至本中心之空間場勘。
(8) 若需專人維護或操作，請補充於維護費預估欄內。
(9) 若它校有該設備，可將網頁截圖與連結等佐證資料提供以利審查。
(10) 經預算審查委員會審查通過並核發經費之設備申請人，需協助後續規格書、審標、測試、驗收等採購與進貨事宜。
(11) 請於收件期限內提出申請並補件完成，逾時不予受理，謝謝配合。

Proposal Process Guidelines:
For those proposing the acquisition of new instruments or the expansion of existing instrument specifications within the Center:
(1) Proposed items must be for "university-wide use." Proposals intended for specific departments (teaching), centers, laboratories, or individual use are not eligible for this application process.
(2) Please consult the existing instrument list on the Precious and Common Instrument Center website under the Office of Research and Development.
(3) Complete the "Proposal Application Form for Precious and Common Instruments of Tzu Chi University."
(4) Submit the application form (Word format), quotation, and supporting documentation via email to the designated contact person.
(5) Submit a hard copy of the application form, signed by co-applicants, department heads, and the Dean, to the Precious and Common Instrument Center in Building D603, Qin-Geng Building.
(6) When applying for the expansion of existing instruments, please ensure that the specifications are compatible with current equipment to avoid procurement of unusable items.
(7) Provide electronic catalogs and quotations from at least three reference vendors. Additionally, include other costs required for instrument installation or normal operation, such as facility renovation and water/electricity configuration. A site inspection of the Center is required prior to submission.
(8) If specialized maintenance or operation is required, please include these details in the estimated maintenance cost column.
(9) If the equipment is available at other universities, please provide supporting evidence, such as website screenshots and links, to facilitate the review process.
(10) Applicants whose equipment proposals are approved and funded by the Budget Review Committee are required to assist with subsequent procurement and delivery procedures, including the preparation of specifications, bidding, testing, and acceptance.
(11) Please ensure that applications and all required supplementary documents are submitted within the deadline. Late submissions will not be accepted. Thank you for your cooperation.
