【校外獎學金】115年度台灣蓁心醫教公益協會委託慈濟大學代辦本校學生參與偏鄉及部落服務獎學金

一、 重要提醒：
1. 本項獎助金係依據《台灣蓁心醫教公益協會委託慈濟大學代辦本校學生參與偏鄉及部落服務
獎學金》辦理。
2. 申請時程：
◎115-1申請時程：115年08月01日-115年11月30日止。
三、申請對象及資料繳交地點與窗口：凡該校在學之護理科及護理系學生，品學合於規定標準且未領取他項獎學金與公費者。

四、申請資格：
1.學業（智育）成績須達65分（含）以上，德育（操行）成績須達70分（含）以上，且無小過（含三支申誡累計）以上處分紀錄（含銷過前紀錄）。
2.申請學生須參加偏鄉或部落的志工服務；志工計算時間為當學期02月01日至07月31日和08月01至01月31日止。
五、應繳資料：（繳交前請詳閱並確認資料正確）
1.學生系統申請書（含同意書）—學生本人務必簽名。
2.偏鄉或部落志工服務活動的時數證明文件。。
3.參與服務活動之工作內容說明一份，需檢附佐證照片（格式不限）。
4.導師或社團指導老師或志工服務隊帶隊老師之推薦函一封。
5.其他有利於審核的相關文件可檢附提供參考，如清寒證明或獲獎資料等。

I. Important Reminders:
1. This scholarship is administered by Tzu Chi University on behalf of the Taiwan Zhenxin Medical Education Charity Association for students participating in rural and tribal service programs.
2. Application Period:
◎ 115-1 Semester: From August 1, 2026, to November 30, 2026.

III. Eligible Applicants and Submission Details:
Open to currently enrolled students in the Department of Nursing and the Nursing Program who meet the specified academic and conduct standards and are not recipients of other scholarships or government-funded grants.

IV. Eligibility Requirements:
1. Academic performance must be 65 points or above, and conduct evaluation must be 70 points or above. Applicants must have no record of minor violations (including a cumulative total of three warnings) or higher (including prior records before expungement).
2. Applicants must participate in volunteer service in rural or tribal areas. Volunteer hours are calculated from February 1 to July 31 and August 1 to January 31 of the current academic year.

V. Required Documents: (Please review and verify all information before submission)
1. Application Form from the Student Information System (including the Consent Form) — must be signed by the student.
2. Official certification of volunteer service hours in rural or tribal areas.
3. A description of the duties performed during the service activities, accompanied by supporting photographs (format unspecified).
4. One letter of recommendation from a class advisor, club advisor, or the leader of the volunteer service team.
5. Any other relevant documents that may support the evaluation, such as proof of financial need or award certificates.
