各位教師 您好：

依據《國家科學及技術委員會補助專題研究計畫作業要點》第十一點第四款規定，研究計畫如涉及人體研究或人類研究，且申請之研究倫理審查單位要求檢附「校送件證明」，請下載「慈濟大學研究倫理審查送審情形填報表（1150401更新）」電子檔，完整填寫後提供予學術研究組，以利後續開立校送件證明。

另因目前正值計畫申請期間，為校送件證明申請高峰期，證明開立作業約需 2 個工作天（不含例假日及國定假日），請務必一併檢附完整填妥之「慈濟大學研究倫理審查送審情形填報表（1150401更新）」電子檔；若僅提供計畫中、英文名稱而未檢附填報表，將無法開立校送件證明，相關作業時程亦將順延。如有時程上的急迫需求，敬請提早備妥相關資料並提出申請，以免影響後續計畫申請及送件時程。

此外，請務必確認研究計畫之中、英文計畫名稱及相關資料均已定稿且不再修改後，再提供電子檔，以免因資料異動而影響文件開立時程。另提醒，若研究計畫涉及人體研究或人類研究，務必申請研究倫理審查（IRB），並預留充足作業時間，儘早準備相關文件及提出申請，以免影響後續計畫申請及送件時程。

**如送審之研究倫理審查單位(非本校簽約)且要求由本校發函，請先洽研究發展處學術研究組協助辦理發文事宜，流程預計7個工作天左右(不含例假日及國定假日)。

▲提醒~研究計畫中涉及以下事項者，應檢附倫理審查委員會相關核准文件(或人類研究倫理審查相關核准文件)。核准文件未能於申請時提交者，須申請時附上已送審之證明文件(非本校開立之校送件證明)。
1.涉及從事人體研究、人體試驗、人類胚胎、胚胎幹細胞研究者。
2.涉及以個人或群體為對象，使用介入、互動之方法、或使用可資識別特定當事人之資料，而進行與該個人或群體有關之系統性調查或專業學科的知識性探索活動者。

★本校未設置研究倫理審查委員會，亦無法開立研究倫理審查免審證明。凡研究計畫涉及人體研究或人類研究者，請依相關研究倫理審查單位之規定，辦理研究倫理審查送審事宜。

Dear Faculty Members,

In accordance with Article 11, Paragraph 4 of the "Operational Guidelines for Research Projects Funded by the National Science and Technology Council," research projects involving human subjects or human research that require a "University Submission Certificate" from the relevant Institutional Review Board (IRB) must complete the "TCU Research Ethics Review Submission Form (Updated 1150401)." Please download the electronic file, complete it in full, and submit it to the Academic Research Section to facilitate the issuance of the University Submission Certificate.

As we are currently in the peak application period for research projects, please be advised that the processing of certificates takes approximately two working days (excluding weekends and public holidays). It is mandatory to attach the fully completed electronic version of the "TCU Research Ethics Review Submission Form (Updated 1150401)." Please note that providing only the Chinese and English project titles without the required form will result in the inability to issue the University Submission Certificate, which will subsequently delay the overall processing timeline. If you have urgent deadlines, please prepare the necessary documents and submit your application well in advance to avoid impacting the project application and submission schedule.

Furthermore, please ensure that the Chinese and English project titles and all related information are finalized and will not be modified before submitting the electronic files, as any subsequent changes may delay the issuance of documents. We also remind you that if a research project involves human subjects or human research, you must apply for Institutional Review Board (IRB) approval. Please allow sufficient time for the review process and prepare the necessary documentation early to avoid affecting the project application and submission timeline.

**If the IRB requiring the submission is not among the university is contracted institutions and requires an official university letter, please contact the Academic Research Section of the Office of Research and Development for assistance with the official correspondence process. This process is expected to take approximately seven working days (excluding weekends and public holidays).

▲ Reminder: Research projects involving the following matters must attach the relevant approval documents from the Institutional Review Board (or human research ethics review approval documents). If the approval documents cannot be submitted at the time of application, proof of submission (other than the University Submission Certificate issued by this university) must be provided.
